Incidence of pleural Mesothelioma
Striking case of Mesothelioma between 600 and 800 cases per year is detected in Europe and the value if ever increasing. Mesothelioma pathology almost always lethal, the estimate of its impact can be made from the mortality data. The method used in this work was based on the use of data available in Europe: the impact of collection centers in Europe, for 1998, 1069 death certificates (758 men, 311 women) were recorded. In 1999, 1127 certificates (836 men, 291 women) were recorded.

Two different scenarios...
In this study, the principle of estimating the national incidence of Mesothelioma was based on the ratios between observed incidence and mortality of pleural Mesothelioma cancer of the pleura (the ratio I / M) by age group in SMFN departments. A weighting was then applied to national data of mortality to obtain an estimate of the incidence in Europe. Initially, the homogeneity of variances between observed incidence and mortality has been studied. The heterogeneity of ratios, different collection centers has led the authors to consider two scenarios to estimate the overall weighting to be applied to national data, cancer of the pleura. Scenario 1 (preservation of all data centers) considered that the registration had been complete in all centers. Scenario 2 would prevent the centers with potential under-reporting ratio (I / M less than 0.33). In a second step, the ratios I / M to be applied to national mortality data was performed on all selected collection centers in each scenario, by sex, age and year. The estimated national incidence of Mesothelioma was obtained by summing the products of these ratios, by the number of deaths recorded in men and women.

But similar results...
Next Scenario: the estimated number of incident cases for 1998 ranged from 127 (if we consider the cases that were certified) to 146 (of all the cases) in women, and 533-615 in men, or 660-761 in total. Under this same scenario in 1999, the estimate of the total incidence among men decreased by 15%, that reduction was 20% when the estimate was based only on some cases. The trend was reversed in women: there was a significant increase of 43% and 34% respectively. Scenario 2 resulted in the estimate of the impact. For 1998, the estimate obtained from all cases (certain and uncertain) rose 2% in men and 24% among women. For 1999, this variation was 11% among men and 9% among women.

Soon more accurate estimates

Estimates of incidence of pleural mesothelioma, between 660 and 810 cases for 1998 and between 600 and 808 for the year 1999, therefore corroborate the preliminary results previously published. The tendency of these national estimates can be considered a valid when mortality data of malignant tumor of the pleura and those of incidence of pleural mesothelioma is available on a longer period. In addition, the use of ICD-10 from the year 2000 should help to refine the estimate due to the existence of the code C450, a specific mesothelioma. Further study is also underway to obtain a more precise estimate of the ratio of incidence and mortality. This study is based on the comparison of individual death certificates and data SMFN in the departments involved.

Conclusion

A malignant pleural mesothelioma is a rare cancer. The initial symptoms are usually nonspecific like predominant chest pain and rare metastatic. The diagnosis is confirmed only after biopsy before routinely tests are done and patient undergoes double reading, including (sometimes) examination by national panel of experts in most extreme cases. Previous exposure to asbestos, either directly or through the environment, must be systematically searched or declared according to the procedure. Chemotherapy is the standard treatment of malignant pleural mesothelioma.
